Does Insurance Cover Physical Therapy in Florida?
Quick Answer
Most plans cover medically necessary outpatient physical therapy, with copays, coinsurance, deductibles, and visit limits that depend on the policy. At All In One Specialty Health in Miami Gardens we review benefits before care starts. We help you submit authorization when your plan requires it. We do not advertise a 24–48 hour insurance decision. Florida direct access can let you begin without a physician referral for up to 30 days even when the insurer still wants a referral to pay.

- Commercial plans: Aetna (HMO/PPO), Ambetter, AmeriHealth Caritas, AvMed, Cigna (HMO/PPO), Florida Blue (PPO), Humana (PPO), Optum Health, Oscar Health, UnitedHealthcare (HMO/PPO), and Self Pay — coverage still depends on your specific contract
- Medicare Part B for outpatient PT when medically necessary, and Medicare Advantage plans from Aetna (HMO), AvMed, CarePlus, Devoted Health, Florida Complete Care, Preferred Care Partners, and Solis Health
- Florida PIP after a motor-vehicle accident — see our PIP patient guide (14-day rule, $10,000 medical benefits, typically 80% of approved PT)
- Workers' compensation when the injury is accepted as work-related
- LOP (letter of protection) cases
We do not accept Medicaid, including Florida Medicaid managed-care plans. Mental health counseling accepts only Medicare and Self Pay for now. Full list: insurance we accept.
How authorization actually works here
Some plans pay from the evaluation with no prior auth. Others require the therapist's plan of care to be authorized after the first visit. Our front desk collects your member ID, checks in-network status, and tells you what we know about copays and remaining visits. Payer clocks vary. Treat “24–48 hours” claims on the internet as marketing, not a promise from this clinic.

Self-pay
If you are out of network, waiting on a referral, or prefer not to bill insurance, ask about self-pay rates when you call. We will not guess a price in this article because it depends on the visit type.
